Sunday Announcement Form

Use this form to enter an announcement to be made Sunday morning from the pulpit. You may also use it for bulletin, website, social media and newsletter announcement requests. Deadline for bulletin announcement is Thursday at 12pm prior to the appropriate Sunday. inclusion of last minute Sunday morning announcement are based on time available and not guaranteed to be made.  

Building Usage Information

Use this document to read the church policy on building usage. The Calendar Request Form is the application for building usage also. Church members and non-members may apply. Decisions are made by the Pastor and church leadership on a per-request basis. 

Calendar Request Form

Use this form if you would like to reserve a date and time on the calendar for a class, event, meeting, etc. 

Reimbursement Request Form

Use this form if you are requesting reimbursement for a church approved purchase using non-church funds (personal check, credit cards, etc.). Receipts or proof of purchase must accompany this form. PLEASE NOTE: Any tax charges are not reimbursed unless approved. 

Authorization Form for Monthly Giving

Use this form to request a set monthly draft of your bank account to go towards tithes or offering.
